JAPAN VISA REQUIREMENTS

GENERAL REQUIREMENTS

Duly accomplished and signed application form (must be typewritten and printed in A4 size paper)
1 pc passport size colored picture (white background)

Original passport (6 months valid)
Original Old passport with previous Japan Visa (if applicable)
Original bank certificate (issued within the last 3 mos upon application) with 6 months Average Daily Balance or ADB (If the bank certificate has no ADB, a 6 months bank statement is required)
Copy of latest income tax return (form 2316 with signature of employee and employer; for business ITR, please attach copy of receipt from BIR)
Original PSA authenticated birth certificate (issued within a year)
Original PSA marriage certificate (issued within a year – if married)
2 Original signed authorization letters (for visa filing and pick-up)
Daily schedule in Japan
Tourism Visa Checklist (Single Entry / Multiple Entry), must be printed back-to-back in A4 size paper, and with original signature

SPECIAL REQUIREMENTS

FOR EMPLOYEES

Original employment certificate (must include applicant’s position, date hired, compensation, office address, landline numbers and e-mail address)

FOR BUSINESSMEN

FOR MINORS

FOR RETIREE

Copy of senior citizen card or retirement documents
Original letter of guarantee if senior is dependent signed by spouse, children or sponsor

FOR FOREIGN NATIONALS

IF THE COMPANY WILL SHOULDER THE TRIP

Letter of guarantee signed by the sponsor
Copy of guarantor or sponsor’s valid ID
Original bank certificate of the company
Copy of business documents and ITR of the company (If employed, company alphalist. If self-employed, receipt from accredited bank of BIR).

ADDITIONAL REQUIREMENTS

If the trip will be shouldered by another person unrelated to the applicant, a proof of relationship is required e.g. pictures together
If the applicant has a used Japanese visa, he will be exempted to submit a birth certificate and/or a marriage contract
If the birth certificate from PSA is unreadable, or if there is no record, submit the below as additional document/s:
Original certificate of non-record and;
Copy of Birth certificate from the local civil registrar
If the birth certificate is late registered, submit the below as additional document/s:
Baptismal certificate
School record (form 137) or school yearbook (if applicable)

SPECIAL NOTES

If the trip will be shouldered by a family member, a proof of relationship and general requirements, including employment certificate / business documents of the guarantor is required.
Additional requirements may be required by the embassy without prior notice.
Personal appearance is not required (unless the Embassy request for it).
